Colorectal cancer is a major cause of morbidity and mortality in Western countries, despite the fact that it is largely preventable through proper screening. Screening tests that can reliably detect both precancerous polyps and early cancers can have a substantial positive impact. Colonoscopy is the undeniable therapeutic gold standard but also currently serves as the predominate initial screening test in the U.S., despite its high costs and invasiveness. CT colonography (virtual colonoscopy) has been shown to be equivalent or even superior to conventional colonoscopy for the detection of advanced neoplasia, but also has the benefits of reduced programmatic costs and invasiveness, as well as opportunistic “extracolonic” abdominal screening. This lecture will consider the use of CT colonography for population screening.
