Iron Supplementation of Blood Donors Dr Ritchard Cable Scientific Director American Red Cross 1300 -1400 Seminar Rooms Strangeways Research Laboratories, Cambridge. ALL WELCOME LUNCH AT 1230 . This talk will review the causes of iron deficiency in blood donors and donor populations at risk. It will summarize findings from HEIRS (Hemoglobin and Iron Recovery Study) and STRIDE (Strategies to Reduce Iron Deficiency in Donors) which were conducted in the United States by REDS -III. Finally it will consider the impact of increased interdonation interval on blood supplies and iron supplementation in donor management. HEIRS showed that providing daily Iron supplements accelerates hemoglobin recovery after donation in donors with baseline ferritin < 50 ng/ml and significantly increases iron recovery. This effect is largely confined to the first 8 weeks after blood donation. Donors without iron supplementation had delayed recovery of iron stores beyond 24 weeks. STRIDE showed that providing regular blood donors with 19 or 38 mg daily iron for 8 weeks or alternatively providing ferritin measurement and recommendations were effective and mostly equivalent strategies for mitigating iron deficiency. Donor compliance was good and side effects minimal and no different from placebo. Control donors without intervention had worsened iron deficiency with continued donation. Based on REDS -II data, extension of the current US 8 week interval to 12 weeks would reduce red cell component donations by 8% and extension to 16 weeks by 13%. Recovery of iron stores in the general REDS -II donor population was estimated to exceed 52 weeks based on multivariable models. Because of these impacts and on the relative ineffectiveness of delaying donation, iron supplementation appears the most direct way to address iron depleted donors.
