 n from suicidal thoughts to suicidal attempts*\n\n
 *Background:* Suicide and self-harm are major publ
 ic health concerns with complex aetiologies which 
 encompass a multifaceted array of risk and protect
 ive factors.  There is growing recognition that we
  need to move beyond psychiatric categories to fur
 ther our understanding of the pathways to both. Al
 though there have been many advances in our unders
 tanding of suicide risk\, our ability to predict s
 uicidal behaviour remains no better than chance. N
 ew approaches to predicting suicidal behaviour foc
 us on distinguishing between those who think about
  suicide and those who attempt suicide.\n\n*Method
 s:* Although a comprehensive understanding of thes
 e determinants of suicidality requires an apprecia
 tion of biological\, psychological and social pers
 pectives\, the focus in this presentation is prima
 rily on the psychological determinants of self-har
 m and suicide. The Integrated Motivational–Volitio
 nal (IMV) Model of Suicidal Behaviour (O’Connor\, 
 2011\; O’Connor & Kirtley\, 2018) provides a frame
 work in which to understand suicide and self-harm.
   This tripartite model maps the relationship betw
 een background factors and trigger events\, and th
 e development of suicidal ideation/intent through 
 to suicidal behaviour. \nFindings: We propose that
  defeat and entrapment drive the emergence of suic
 idal ideation and that a group of factors\, entitl
 ed volitional moderators\, govern the transition f
 rom suicidal ideation to suicidal behaviour.  Acco
 rding to the IMV model\, volitional moderators inc
 lude access to the means of suicide\, exposure to 
 suicidal behaviour\, capability for suicide (fearl
 essness about death and increased physical pain to
 lerance)\, planning\, impulsivity\, mental imagery
  and past suicidal behaviour.  I will present a se
 lection of empirical studies derived from the IMV 
 model to illustrate how psychological factors incr
 ease suicide risk and what can be done to ameliora
 te such risk. \n\n*Discussion:*  The IMV model ide
 ntifies 8 key pillars that govern the transition f
 rom suicidal thoughts to suicide attempts.  Clinic
 al interventions need to target the volitional pha
 se factors to reduce the likelihood that suicidal 
 thoughts are acted upon. The implications for the 
 prevention of self-harm and suicide will also be d
 iscussed. \n
