The public health benefits and feasibility of canine rabies elimination through mass dog vaccination have been established. However the reality is that in most developing countries rabies is endemic. Bites by rabid dogs kill thousands of people every year and the delivery of life-saving post-exposure vaccines costs millions of dollars. I present insights into the epidemiological dynamics of rabies from detailed studies in endemic settings in East Africa and their relevance in the applied context of rabies control. I examine the impacts of control efforts from a range of settings around the world and summarise lessons for future rabies control and elimination programmes.
